Two Shell Plaza 777 Walker Houston, TX 77252 CPF 4-2010-5017H Dear Mr. Smith: On November 19, 2010, Shell Pipeline Company, LP (Shell) experienced a failure of its 22-inch diameter Houma-to-Houston crude oil pipeline near Vinton, LA. As a result of the accident the Pipeline and Hazardous Materials Administration (PHMSA) issued a Corrective Action Order (CAO), 4-2010-5017H, dated November 23, 2010, requiring Shell to take corrective actions with respect to its pipeline system. Shell performed all actions as required by this Order. PHMSA monitored Shell’s actions to determine that Shell has substantially completed the requirements of the CAO.
