As a result of the inspection, Chaparral was issued a Notice of Amendment on October 31, 2013. Chaparral submitted the requested procedural modifications on December 3, 2013. My staff reviewed the amended procedures, and requested additional changes on February 13, 2014. The additional changes were received in our office on February 19, 2014. My staff reviewed the amended procedures, and it appears that the inadequacies outlined in this Notice of Amendment have been corrected.
