420155014M_Closure Letter_11122015_text.pdf, page 1Official PDFCERTIFIED MAIL - RETURN RECEIPT REQUESTED November 12, 2015 Mr. Todd Denton President Phillips 66 Pipeline LLC 3010 Briarpark Drive Houston, TX 77042 CPF 4-2015-5014M Dear Mr. Denton: After Phillips 66 Pipeline LLC (Phillips) submitted Safety Related Condition Report Number 2014-0149 on December 14, 2014, representatives of the Pipeline and Hazardous Materials Safety Administration (PHMSA) pursuant to Chapter 601 of 49 United States Code initiated an inspection of the Phillips Integrity Management Plan. PHMSA inspected procedures that are used for determination of seam failure susceptibility and assessment intervals as part of Phillips’ Integrity Management Plan. As a result of the inspection, Phillips was issued a Notice of Amendment (NOA) on July 2, 2015, which proposed amendment of your procedures. On August 3, 2015, Phillips responded to the NOA, and requested a hearing. In a letter dated November 5, 2015, Phillips rescinded its request for a hearing on this matter and submitted the requested procedural modifications. My staff reviewed the amended procedures, and the inadequacies outlined in this Notice of Amendment have been corrected.
