Box 100360 Anchorage, AK 99510 CPF 5-2008-5030M Dear Mr. Isaacson: On July 14-17, 2008, a representative of the Pipeline and Hazardous Materials Safety Administration (PHMSA), pursuant to Chapter 601 of 49 United States Code, inspected your Ohktok pipeline facihties in Prudhoe Bay, Alaska. As a result of the inspection, it appears that you have committed a probable violation of the Pipehne Safety Regulations, Title 49, Code of Federal Regulations. The items inspected and the probable violations are: (e) Each valve other than a check valve must be equipped with a means for clearly indicating the position of the valve (open, closed, etc. ).#
