520116011W_warning letter_12072011_text.pdf, page 1Official PDFWARNING LETTER VIA: UPS GROUND December 7, 2011 Mr. Thomas Mushovic General Manager Aircraft Service International Group 6000 DeHavilland Drive Anchorage, AK 99502-0246 CPF 5-2011-6011W Dear Mr. Mushovic: On November 7-9, 2011, a representative of the Pipeline and Hazardous Materials Safety Administration (PHMSA), pursuant to Chapter 601 of 49 United States Code, inspected Aircraft Service International Group’s (ASIG) pipeline facilities in Anchorage, Alaska. As a result of the inspection, it appears that you have committed a probable violation of the Pipeline Safety Regulations, Title 49, Code of Federal Regulations. The items inspected and the probable violation is: 1. §195.426 Scraper and sphere facilities. No operator may use a launcher or receiver that is not equipped with a relief device capable of safely relieving pressure in the barrel before insertion or removal of scrapers or spheres. The operator must use a suitable device to indicate that pressure has been relieved in the barrel or must provide a means to prevent insertion or removal of scrapers or spheres if pressure has not been relieved in the barrel. During the field inspection, PHMSA’s inspector observed and documented that the safety relieving device on the closure at the pig receiver at ASIG’s Airport Facility was disabled.#

We have reviewed the circumstances and supporting documents involved in this case, and have decided not to conduct additional enforcement action or penalty assessment proceedings at this time. We advise you to correct the item identified in this letter. Failure to do so will result in Aircraft Service International Group being subject to additional enforcement action. No reply to this letter is required. If you choose to reply, in your correspondence please refer to CPF 5-2011-6011W and send all documents to our office at 188 W.
