Cause detail: PUMP OR PUMP-RELATED EQUIPMENT
Cause detail: LOCAL OPERATION PERSONNEL DISCOVERED APPROXIMATELY 1.9 BARRELS OF CRUDE OIL INSIDE LINED CONTAINMENT AREA. INVESTIGATION REVEALED THE PUMP'S 6" BALON ANSI 150 SUCTION VALVE BODY GASKET FAILED CAUSING A DRIZZLE LEAK OF CRUDE OIL. CONTRACTOR WAS MOBILIZED FOR CLEANUP, RECOVERING ALL OF THE LEAKED PRODUCT FOR DISPOSAL. FAILED VALVE WAS REMOVED WITH BLIND FLANGE INSTALLED. DETERMINATION WAS MADE THAT THE PUMP IS NO LONGER NECESSARY AND IT WAS MADE AVAILABLE FOR POSSIBLE FUTURE USE AT ANOTHER LOCATION.
