Cause detail: BODY OF PIPE
Cause detail: THE CRACK INITIATED AT THE WELD TOE OF THE BRANCH CONNECTION AND PASSED THROUGH A PORTION OF THE HEAT-AFFECTED ZONE THAT EXTENDED INTO THE CARRIER PIPE. THE CAUSE OF THE FAILURE WAS THE COMBINED WEIGHT OF THE BLIND FLANGE, SIDE TAP VALVE AND SOIL OVERBURDEN ACTING AS AN UNSUPPORTED, CANTILEVERED LOAD ON THE BRANCH CONNECTION.
