Cause detail: ON NOVEMBER 21, 2015, THE 8-INCH CARSON LATERAL WAS PUNCTURED ON THE SOUTH SIDE OF MICA DRIVE AT US-395 DURING AN IN-LINE INSPECTION (ILI) TOOL RUN. THE PUNCTURE OCCURRED AT A DROP SECTION CONTAINING 45-DEGREE ELBOWS INSTALLED IN 1987 AS PART OF A RELOCATION FOR THE CONSTRUCTION OF MICA DRIVE. AN INVESTIGATION ON THE CAUSE OF THE INCIDENT HAS BEEN COMPLETED AND IT WAS DETERMINED THAT DURING THE ILI OF THE CARSON LATERAL, THE LEADING EYE OF THE MFL TOOL STRUCK THE INSIDE OF THE 45 DEGREE ELBOW AND CAUSED A PUNCTURE IN THE PIPELINE. COMPANY EMPLOYEES AND CONTRACTORS WERE STATIONED AT THE SEGMENT ISOLATION VALVES AS PART OF THE ILI PROCEDURE ALLOWING FOR A RAPID RESPONSE AND SHUTDOWN OF THE PIPELINE AS PART OF THE ISOLATION PLAN. PUBLIC AND PRIVATE PROPERTY DAMAGE INCLUDES SIX VEHICLES AND A LANDSCAPE RETAINING WALL AS A RESULT OF THE BLOWING DIRT AND ROCKS. THE MAGNETIC FLUX LEAKAGE ILI TOOL WAS ALSO DAMAGED.
