Cause detail: PUMP OR PUMP-RELATED EQUIPMENT
Cause detail: GLAND NUT ON A PUMP FAILED RESULTING IN A LOSS OF 2 BBLS DUE TO STRIPPED THREADS AND PACKING SLID OUT. THE FAILURE WAS IDENTIFIED BY LOCAL OPERATING PERSONNEL. THE CONTROL CENTER WAS IMMEDIATELY CONTACTED THE UNIT SHUTDOWN. THE RELEASED CRUDE OIL WAS CONFINED TO OPERATOR PROPERTY AND CLEANED UP PROMPTLY. THE PUMP WAS DISABLED AND REPAIR IS ONGOING.
