Cause detail: INTERNAL CORROSION
Cause detail: A THIRD-PARTY PUMPER NOTIFIED NUSTAR OPERATIONS PERSONNEL OF A CRUDE OIL RELEASE AT NUSTAR'S OXY-SHIELDS LACT FACILITY. THE LEAK ORIGINATED AT AN ABOVE-GRADE 6-INCH VALVE FLANGE CONNECTION BETWEEN NUSTAR'S OXY-SHIELDS LATERAL AND THE LACT FACILITY AND WAS CAUSED BY INTERNAL CORROSION OF THE FLANGE FACE. THE UPSTREAM PIPELINE WAS SHUT DOWN, AND THE LEAKING PIPE SEGMENT WAS ISOLATED. THE PIPE SPOOL CONTAINING THE FLANGE WAS REPLACED, AND A NEW GASKET WAS INSTALLED.
