Cause detail: THREADED CONNECTION/COUPLING FAILURE
Cause detail: A DISCHARGE SWEDGE FAILED DUE TO A HAIRLINE CRACK ON A 20 HP PUMP AT THE CHORD OWNED OM ERICKSON 7-6 RESULTING IN 3.3 BBLS OF CRUDE OIL RELEASED ONTO THE LEASE SITE. UPON INVESTIGATION, IT WAS DETERMINED THAT PRODUCER OWNED LACT UNIT STRAINER WAS CLOGGED, RESULTING IN CAVITATION OF KM PUMPS LEADING TO VIBRATION THAT CRACKED THE SWEDGE. CLEAN-UP IS ONGOING.
