Cause detail: BOLT ON SLEEVE THAT WAS INSTALLED IN FEBRUARY WAS FOUND LEAKING (SMALL LEAK BUBBLING IN WATER). AFTER EXAMINATION IT WAS DETERMINED THAT THE LEAK COULD NOT BE REPAIRED SO A REPLACEMENT OF PIPE WILL BE BE INSTALLED. 12-18-13 REPLACEMENT OF IMPACTED SEGMENT OF PIPE WAS COMPLETED ON JULY 23, 2013. 3-17-14 REVISED SECTION G8 TO CORRECT SUB-CAUSE DESCRIPTION. 4-9-14 REVISED SECTION G8 AT PHMSA'S REQUEST.
