Cause detail: ON 09/25/25 AT APPROXIMATELY 10:24, CONTRACTORS WORKING FOR THE COMPANY PUNCTURED THE 21002.01 - 26INCH NEDERLAND TO TRINITY LINE CAUSING CRUDE OIL TO RELEASE. OPERATIONS WAS IMMEDIATELY NOTIFIED AND DISPATCHED TO THE LOCATION. THE LINE WAS ISOLATED BECAUSE THE LINE WAS ALREADY SHUT DOWN FOR A PROJECT. APPROXIMATELY 25 BARRELS OF CRUDE OIL WAS RELEASED TO THE SOIL WITH 5 GALLONS MIGRATING INTO LNVA CANAL. AN OSRO CLEANED UP RELEASED PRODUCT AND CONTAMINATED SOIL. THE LEAK WAS REPAIRED BY INSTALLING A MECHANICAL CLAMP. THE LINE WAS RESTARTED ON 09/26/25 AT APPROXIMATELY 09:00. THE CAUSE OF THE ACCIDENT IS STILL UNDER INVESTIGATION. A SUPPLEMENTAL REPORT WILL BE SUBMITTED ONCE COMPLETE.
