Cause detail: THIRD PARTY EXCAVATION DAMAGE

Cause detail: A CONTRACTOR, EMPLOYED BY THE NORTH CAROLINA DEPARTMENT OF TRANSPORTATION, WAS DRIVING PILINGS NEXT TO A BRIDGE CROSSING AHOSKIE CREEK AS PART OF BRIDGE MAINTENANCE PROJECT. THE 12-INCH TRANSMISSION PIPELINE, WHICH HAD BEEN INSTALLED IN 2002 WITH A HORIZONTAL DIRECTIONAL DRILL, WAS AT A DEPTH OF APPROXIMATELY 23 FEET WHEN IT WAS STRUCK BY ONE OF THE PILINGS. THE NORTH CAROLINA ONE CALL CENTER HAD NOT BEEN NOTIFIED PRIOR TO THE WORK AND THEREFORE THE PIPELINE HAD NOT BEEN LOCATED. A PERMANENT PIPELINE MARKER WAS INSTALLED ON ONE SIDE OF THE CREEK CROSSING NEXT TO THE PAVEMENT.
