Cause detail: ON MARCH 27, 2024, AT APPROXIMATELY 8:11 A.M., OPERATING PERSONNEL DISCOVERED A 1/2-BARREL CRUDE OIL RELEASE ORIGINATING FROM THE BARREL DOOR ON A LAUNCHER TRAP. THE APPARENT CAUSE OF THE RELEASE WAS THE FAILURE OF THE O-RING ON THE TRAP DOOR FROM HEAT EXPANSION. THERE WERE NO INJURIES, AND THE SITE WAS CLEANED UP AND REPAIRED PROMPTLY.
