AN INSPECTION HAS INITIALLY BEEN COMPLETED BUT FURTHER INVESTIGATION IS NECESSARY TO RESOLVE THIS INCIDENT AND WILL BE FORWARDED AS A SUPPLEMENTAL REPORT. A MEETING WAS HELD WITH ED CLARKSON,DAVE HIPPCHEN AND DARRELL MCKOWN OF THE WV PSC ON MAY 24, 2002.
