Cause detail: THREADED CONNECTION/COUPLING FAILURE

Cause detail: INVESTIGATION OF A LEAK CONFIRMATION ON A 16" OFFSHORE GATHERING PIPELINE OWNED BY TRANSCO, WAS DETERMINED TO BE A 1" PLEECO BLEEDER PORT PLUG. ATTEMPTS TO STOP THE LEAK BY TIGHTENING THE PLUG DID NOT WORK. THE PLUG WAS REMOVED AND REPLACED WITH A1" STAINLESS STEEL BALL VALVE AND PLUG. THE LINE WAS PRESSUREIZED, CHECKED FOR LEAKS. THE LINE WAS PUT BACK INTO SERVICE.
