Cause detail: DESIGN-, CONSTRUCTION-, INSTALLATION-, OR FABRICATION-RELATED
Cause detail: A PINHOLE LEAK WAS FOUND IN A GIRTH WELD DURING A ROUTING ANNUAL LEAK SURVEY. THE GAS LEAKAGE WAS MINIMAL. THE PIPELINE REMAINED IN SERVICE DURING A COLD SPELL, AND THEN IMMEDIATELY TAKEN OUT OF SERVICE. AFTER THE SECTION WAS REPLACED, IT WAS FOUND THAT THE WELD HAD AN INCOMPLETE ROOT PASS AND HAD A LESS THAN 3", HAIRLINE FRACTURE UNDERNEATH THE PINHOLE.
