Cause detail: NON-THREADED CONNECTION FAILURE
Cause detail: AT 8:15 AM ON 04/25/2024, AN SCM OPERATIONS TECHNICIAN IDENTIFIED A LEAK COMING FROM A 10" CLOSURE ON THE ABOVE GROUND PIG RECEIVER. THE CAUSE OF THE LEAK WAS IDENTIFIED TO BE A DAMAGED O-RING ON THE CLOSURE. TO REPAIR THE CONNECTION, THE O-RING WAS REPLACED, AND THE CLOSURE WAS PROPERLY SHUT PER MANUFACTURER SPECIFICATIONS. THE CORRECTIVE ACTION TAKEN TO PREVENT RECURRENCE IS TO ADD AN O-RING INSPECTION TO THE MAINTENANCE WORK ORDERS FOR EACH PIG RUN.
