Cause detail: A SECTION OF 1" STEEL TUBING A DRY BED DEHYDRATION UNIT FAILED, CAUSING GAS TO IGNITE AND FORCING THE SHUTDOWN OF THE DEHYDRATION UNIT. 11-4-04 THE HEAT EXCHANGER FAILED DUE TO RAPID OVERHEATING, HOWEVER THE EXACT CAUSE OF OVERHEATING COULD NOT BE DETERMINED BY THE LAB (MODERN INDUSTRIES).
