Cause detail: PUMP OR PUMP-RELATED EQUIPMENT
Cause detail: AN ESTIMATED RELEASE VOLUME OF 15.36 BARRELS OF CRUDE OIL WAS CONTAINED WITHIN A LINED AREA ON THE CONTINENTAL RESOURCES- CLAYTON- ALICE HSU WELL PAD. THE RELEASE OCCURRED DUE TO A ROPER PUMP FAILURE, WHERE THE SHAFT INSIDE THE PUMP SEIZED ON THE BRASS BUSHING. THIS CAUSED THE BRASS BUSHING TO ROTATE AND CREATE A HOLE IN THE PUMP HOUSING. CLEANUP HAS BEEN COMPLETED. TRAINING WAS HELD WITH TEAM MEMBERS THAT OPERATE AND REBUILD ROPER PUMPS.
