Cause detail: NON-THREADED CONNECTION FAILURE
Cause detail: P66 EMPLOYEES WERE PREFORMING MAINTENANCE ON 4" CRUDE GATHERING PIPELINE ATTEMPTING TO DISLODGE PARAFFIN PLUG. MAINTENANCE EMPLOYEES WERE MONITORING GAGE PRESSURE WHEN DROP IN PRESSURE OCCURRED. THE EMPLOYEES STOPPED THE WORK AND STARTED LOOKING FOR POSSIBLE LEAK. THE PIPELINE DEVELOPED A LEAK ON A TEMPORARY CLAMP INSTALLED FOR MAINTENANCE. THE LEAK OCCURRED BETWEEN THE RUBBER SEALING AREA AND THE PIPELINE. THE LEAK RESULTED IN 22.7 BARRELS UNINTENTIONAL RELEASE OF CRUDE OIL AND WATER MIXTURE. THE LEAK WAS STOPPED AND CONTAINMENT AND CLEAN UP WAS ADDRESSED IMMEDIATELY. THE CLAMP WAS REMOVED, AND THE AFFECTED SECTION WAS CUT OUT AND REPLACED WITH NEW PIPE TO RESTORE INTEGRITY. UPDATED: LOCAL CONTACT INFORMATION. UPDATED: PART E - APPARENT CAUSE.
