Cause detail: LOADER CLEANING OUT OF A DRAINAGE DITCH THAT WAS LOCATED ON THE SOUTHWESTERNSTAKES. IT APPEARS THE OPERATOR STRUCK THE 8" LPG PIPELINE WITH A STEEL TOOTHON THE BUCKET OF THE FRONT END LOADER. THERE WAS A RELEASE OF LPG. PATEAND PATE AGAIN FAILED TO NOTIFY COASTAL STATES CRUDE GATHERING COMPANYCONCERNING THE SCOPE OF WORK BEING CONDUCTED NEAR THE 8" LPG PIPELINE ON JULY14, 1998.
