Cause detail: MALFUNCTION OF CONTROL/RELIEF EQUIPMENT
Cause detail: THE RELIEF VALVES ON THE COOLING FANS ARE SET AT 1,500 PSI AND SIX OF THEM VENTED AT 1,471 PSI. WHEN THREE OF THE RELIEF VALVES RELIEVED, THE VENT STACKS SLID DOWN IN THE BRACKET THAT WAS SECURING THEM ON THE TOP FRAME OF THE COOLING FANS. THE DOWNWARD FORCE BROKE THE NIPPLE UNDERNEATH THE RELIEF VALVE.
