Cause detail: THREADED CONNECTION/COUPLING FAILURE
Cause detail: DURING A TRUCK DELIVERY AT ILES STATION FACILITY, THE TRUCK DRIVER NOTICED VISIBLE OIL ON THE GROUND COMING FROM THE PUMP HOUSE. OPERATIONS DETERMINED A 1/2 INCH THREADED FITTING FAILED JUST DOWNSTREAM OF THE PUMP. THE FITTING FAILED DUE TO HIGH VIBRATION AND INCORRECT PIPE SCHEDULE. FITTING HAS BEEN REPLACED AND WORK WAS COMPLETED TO REDUCE VIBRATION ON THE POSITIVE DISPLACEMENT PUMP. ENVIRONMENTAL REMEDIATION IS PLANNED ALTHOUGH SCOPE IS STILL BEING DEVELOPED. 9/2/2022 UPDATE: NO CHANGES
