Cause detail: BOOSTER PUMP MOTOR FAILED
Cause detail: WHILE PUMPING PRODUCT FROM TANKAGE TO AN OUTGOING PIPELINE, A 100 H.P. BOOSTER PUMP MOTOR BEARING FAILED, HEATED UP AND CAUSED THE PUMP SEAL TO FAIL. LEAKING PRODUCT WAS IMMEDIATELY IGNITED. THE PUMP WAS IMMEDIATELY SHUT DOWN, ISOLATED, AND THE FIRE WAS EXTINGUISHED. REPAIRS ARE PRESENTLY UNDERWAY TO REPLACE THE MOTOR AND EXPECT TO BE COMPLETED ON NOVEMBE 16, 1989.
