Cause detail: NON-THREADED CONNECTION FAILURE
Cause detail: ON 11/18/2015 OPERATION PERSONNEL DISCOVERED A CRUDE OIL RELEASE OF APPROXIMATELY 2 BARRELS AT EASTLAND STATION. INVESTIGATION DETERMINED THAT THE RELEASE WAS FROM A STEM SEAL PACKING FAILURE ON A 24" 600# DHV BLOCK VALVE. THE AFFECTED VALVE WAS UNDER WARRANTY AND HAS BEEN REMOVED AND A NEW VALVE HAS BEEN INSTALLED. CONTAMINATED SOIL WAS REMEDIATED ONSITE.
