Cause detail: ON DECEMBER 28, 2015, THE PANHANDLE EASTERN PIPE LINE COMPANY 22-INCH EDGERTON 100 LINE EXPERIENCED AN UNINTENTIONAL IN-SERVICE RELEASE. THE APPROXIMATE LOCATION WAS 18736 SWITZER ROAD, IN DEFIANCE COUNTY, NEAR DEFIANCE OHIO. THERE WERE NO INJURIES, LOCAL OPERATIONS CONFIRMED THAT 25 HOMES WERE EVACUATED WITH A TOTAL OF APPROXIMATELY 50 PEOPLE. A ROOT CAUSE FAILURE ANALYSIS WAS PERFORMED TO DETERMINE THE CAUSE. THIS SUPPLEMENTAL/FINAL REPORT UPDATES PART D - 7.D - ESTIMATED OTHER COSTS AND PART G - G6 - EQUIPMENT FAILURE, BASED ON THE ROOT CAUSE FAILURE ANALYSIS.
