Cause detail: EXCAVATOR FAILED TO MAINTAIN CLEARANCE AFTER VERIFYING MARKS
Cause detail: ON 2/22/2016 A THIRD PARTY OPERATOR DAMAGED THE PIPELINE WHILE INSTALLING DRAIN TILE IN A FARM FIELD. COBRA WAS NOTIFIED OF THE DAMAGE AND DISPATCHED PERSONNEL TO THE SCENE. THE PERSONNEL ASSESSED THE DAMAGE AND GAS SUPPLY WAS ISOLATED AT 3 PM USING A KEY VALVE A FEW HUNDRED YARDS NORTH OF THE INCIDENT AREA AND A KEY VALVE SOUTH OF THE INCIDENT AREA. AT THIS POINT THE LINE WAS MANUALLY REGULATED DOWN TO 80 PSI TO MINIMIZE GAS LOST WHILE ALLOWING UTILITY GAS CUSTOMERS TO MAINTAIN SERVICE. A LINE STOPPLE WAS INSTALLED A FEW HUNDRED YARDS SOUTH OF THE INCIDENT AREA TO FULLY ISOLATE THE DAMAGED SECTION. THIS COMPLETELY STOPPED THE RELEASE OF GAS AND ALLOWED THE REST OF THE SYSTEM OUTSIDE THE INCIDENT AREA TO RETURN TO NORMAL OPERATING PRESSURE WHILE THE REPAIRS WERE MADE.
