Cause detail: THREADED CONNECTION/COUPLING FAILURE
Cause detail: AT 13:50 DURING ROUTINE ROUNDS A OPERATOR DISCOVERED 1/4" SAMPLE VALVE WITH NIPPLE HAD BROKE AT THE NIPPLE. OPERATOR PULLED ESD WHICH SHUT DOWN THE UNIT. FREE STANDING LIQUIDS WERE RECOVERED. CLEANUP OF THE EQUIPMENT AND REMEDIATION PROCESS STARTED. THE FAILED COMPONENT WAS ANALYZED AND FOUND TO BE INSUFFICIENT FOR REQUIRED OPERATING PARAMETERS. THIS RESULTED IN A REPLACEMENT WITH A EXTRA HEAVY NIPPLES IN ALL SIMILAR APPLICATIONS ON THE SYSTEM.
