Cause detail: ON MAY 13, 2024, AT 12:54 P.M., THE OPERATIONS CONTROL CENTER (OCC) RECEIVED AN ENVIRONMENTAL BOX LEVEL SWITCH HIGH HIGH (LSHH) ALARM AT THE COACHWHIP #2 LEASE AUTOMATIC CUSTODY TRANSFER (LACT) UNIT. AT 7:28 P.M., THE LSHH ALARM WAS RECOGNIZED BY THE CONTROLLER, AND LOCAL OPERATING PERSONNEL WERE NOTIFIED OF A POTENTIAL RELEASE. AT 8:23 P.M., OPERATING PERSONNEL ARRIVED ON-SITE AND CONFIRMED A TEN (10) GALLON RELEASE FROM THE ENVIRONMENTAL BOX. THE ENVIRONMENTAL BOX OVERFILLED DUE TO INADVERTENT MISALIGNMENT OF THE NEEDLE VALVE. THERE WERE NO INJURIES, AND THE SITE WAS CLEANED UP PROMPTLY.
