Cause detail: ON MARCH 19, 2019 AT 8:00 AM CST, A SPIRE STREET REPAIR CREW DAMAGED A DRIP BOX WHILE LIFTING A STREET COVER PLATE. THE 2" DRIP RISER ATTACHED TO THE TRANSMISSION PIPELINE WAS CRACKED INSIDE THE DRIP BOX AND RESULTED IN A LEAK. THE TRANSMISSION PIPELINE WAS NOT DAMAGED. SPIRE INSTALLED A 2" STOPPER FITTING AND CAPPED THE RISER TO REPAIR THE LEAK, WHICH WAS COMPLETED AT APPROXIMATELY 12:00 AM CST ON MARCH 21, 2019. A 48 HR. UPDATE WAS CALLED INTO THE NRC AT 9:18 AM CST ON MARCH 22, 2019 THAT THE LEAK HAD BEEN PERMANENTLY REPAIRED AND THERE WAS NO MORE RELEASE OF GAS TO THE ENVIRONMENT; NRC REPORT NUMBER 1240741.
