Cause detail: THIRD PARTY EXCAVATION DAMAGE

Cause detail: A SUB-CONTRACTOR FOR H20 CONSTRUCTION WAS WORKING ON 2ND STREET AS PART OF A STREET WIDENING PROJECT. THEY WERE USING HEAVY EQUIPMENT TO GRADE THE NEW ROADWAY. APPARENTLY, THEY WENT BEYOND THE AREA OF CONSIDERATION AND HIT THE 4" STEEL GAS LINE. THERE WERE 10 HOLES OF VARIOUS SIZES IN THE BODY OF THE PIPE. THE CONTRACTOR DID NOT MAKE ANY REQUESTS FOR UTILITY LOCATES BEFORE WORKING. 4TH AND 2ND STREETS WERE TEMPORARILY CLOSED DURING THE INCIDENT. TWO RESIDENTS WERE TEMPORARILY EVACUATED WHILE THE SITUATION WAS BROUGHT UNDER CONTROL.
