Cause detail: THREADED CONNECTION/COUPLING FAILURE
Cause detail: ON OCTOBER 31, 2023, AT APPROXIMATELY 1:30 P.M., THE OPERATIONS CONTROL CENTER (OCC) WAS NOTIFIED OF A RELEASE AT THE HOLT NORTH LEASE AUTOMATIC CUSTODY TRANSFER (LACT) SITE. APPROXIMATELY 1.5 BARRELS OF CRUDE OIL WERE RELEASED. THE CAUSE OF THE RELEASE WAS A FAILURE OF A 2.5-INCH THREADED UNION ON THE PSV. THERE WERE NO INJURIES, AND THE SITE WAS CLEANED UP AND REPAIRED PROMPTLY.
