Cause detail: ALTHOUGH WE FOUND NO INTERNAL CORROSION OR EXTERNAL PITTING OR CORROSION, THE WALL OF THE PIPE HAD BEEN GREATLY REDUCED. WE SENT THE PIPE TO A METALLURGIST TO DETERMINE WHY THIS THINNING OF THE PIPE OCCURRED. 7-29-03 THE INITIAL REPORT ON THIS INCIDENT WAS SENT ON 9/27/02. A SUBSEQUENT REPORT WAS SENT ON 1/3/03 FOLLOWING A METALLURGICAL STUDY. WE THEN RESENT THE REPORT ON 4/23/03 USING THE NEW FORM. THIS REPORT IS SENT TO ADD REQUESTED DATA THAT WAS LEFT OFF OF THE EARILER REPORTS. PLEASE NOTE THAT WE HAVE UPDATED A COUPLE OF ITEMS INCLUDING THE FINAL COST OF THE INCIDENT.
