Cause detail: ON APRIL 3, 2019 AT 1548 HOURS, PG&E IDENTIFIED A GRADE 1 LEAK ALONG DFM 0609-02 AT 6434 FLORIN RD, SACRAMENTO. PG&E CREWS WERE ONSITE AND FINISHED MAKING REPAIRS TO THE LEAKS DISCOVERED ON A SAVE-A-VALVE AND PCF ON APRIL 4, 2019 AT 0414 HOURS. A LEAK SURVEY WAS CONDUCTED FOLLOWING THE REPAIRS AND FOUND AN ADDITIONAL LEAK ON THE WELD OF DFM 0609-02. ON APRIL 6, 2019, PG&E CUT OUT THE FOLLOWING: LEAKING MITER ELBOW, PCF, SAVE-A-VALVE, 6IN ELBOW AND INSTALLED A NEW 6IN PCF AND MAIN. THE APPARENT CAUSE WAS DETERMINED AS METALLURGICAL FAILURE OF THE MITER WELD, LIKELY DUE A CONSTRUCTION-RELATED DEFECT. THERE ARE NO KNOWN INJURIES OR FATALITIES. CUSTOMER IMPACT IS UNKNOWN. NO POLICE OR FIRE DEPARTMENT REPORTED. ON APRIL 5, 2019, THIS INCIDENT WAS REPORTED TO DOT (1241929) AND CPUC (190405-9141) DUE TO MEDIA PRESENCE (FOX NEWS) AND THE EXPECTED REPAIR COSTS BEING ESTIMATED AT OVER $50K.
