Cause detail: PUMP OR PUMP-RELATED EQUIPMENT
Cause detail: ON JUNE 7, 2023, AT APPROXIMATELY 10:45 AM, IRONWOOD OPERATIONS WAS NOTIFIED BY A THIRD-PARTY LEASE OPERATOR THAT THERE WAS A POSSIBLE LEAK ON A CHARGE PUMP ON THE GATHERING SYSTEM. IRONWOOD OPERATIONS ARRIVED ON LOCATED AT APPROXIMATELY 11:05 AM AND ISOLATED THE CHARGE PUMP. THERE WAS A SMALL AMOUNT OF CRUDE THAT WAS RELEASED, ESTIMATED VOLUME IS .27 BARRELS (11.42 GALLONS), AND WAS PROMPTLY REMEDIATED. THE RELEASE WAS DUE TO THE PUMP'S COUPLING BOLTS BEING SHEARED OFF; THE APPARENT CAUSE IS VIBRATION. THIS ALLOWED THE PUMP SHAFT TO MOVE AXIALLY DUE TO THE FLUID PRESSURE, CAUSING THE LEAK. THE PIPELINE MOP IS 1440, AND THE PRESSURE AT THE TIME OF THE RELEASE WAS 450 PSI.
