Cause detail: ON 9/25/2024 AT 10:34 EDT OPERATIONS AT LEBANON TERMINAL DISCOVERED PRODUCT ON THE GROUND NEAR ABOVE GROUND STORAGE TANK 3361. AN ESTIMATED 70 BARRELS OF TRANSMIX WERE RELEASED. APPARENT CAUSE IS A FAILED FLOOR OR SEAM WELD DUE TO SETTLEMENT. THE TANK HAS BEEN ISOLATED AND AIR GAPPED, AND WAS DECOMMISSIONED ON 9/25/2024.
