Cause detail: NON-THREADED CONNECTION FAILURE

Cause detail: ON 4/25, COMPANY MECHANICS DEPLOYED TO THE SITE OUT TO REPLACE A 4-INCH VALVE AND GASKET AS PART OF NORMAL MAINTENANCE. THEY COMPLETED THE WORK WITHOUT INCIDENT AND RETURNED TO THE SITE TO CONTINUE WORK ON 4/26, WHEN THEY NOTICED A LEAK FROM THE FLANGE GASKET THAT HAD JUST BEEN REPLACED. CORRECTIVE ACTION TO REPLACED GASKET AND RE-TORQUED THE FLANGE IN CONJUNCTION WITH THE CLEANUP OF THE PRODUCT THAT WAS CONTAINED ON THE PAD.
