Cause detail: THREADS STRIPPED, BROKEN PIPE COUPLING
Cause detail: THE RELEASE OCCURRED WHEN AN INTERNAL THERMOWELL BROKE ON THE PUMP#3 ALLOWING VGO TO LEAK THROUGH ELECTRICAL CONDUITS TO AN ELECTICAL PANEL. KPL OPERATIONS PERSONNEL DISCOVERED THE RELEASE AND ACTIVATED KPL'S EMERGENCY NOTIFICATION SYSTEM ON 6/19/05. KPL OPERATIONS PERSONNEL INITIALLY APPLIED ABSORBANT PADS TO THE SPILL AREA. MILLER WAS CONTACTED AND RESPONDED WITH A VACUUM TRUCK AND RECOVERED APPROXIMATELY 1 TO 1.5 BARRELS OF VGO. MILLER THEN USED A BACKHOE TO EXCAVATE VGO IMPACTED SOIL. ON 6/20/05 ARCADIS ARRIVED ON SITE (REMEDIATION CONTRACTOR) AND DIRECTED FURTHER EXCAVATION BY MILLER USING A BACKHOE AND HAND DIGGING. THIRTEEN SAMPLES WERE TAKEN AND THE RESULTS INDICATED THAT FOUR GENERAL AREAS WOULD REQUIRE ADDITIONAL EXCAVATION. THE FOUR AREAS ARE SCHEDULED TO BEGIN ON JULY 15, 2005. AFTER EXCAVATION A SECOND ROUND OF SAMPLES WILL BE CONDUCTED TO DETERMINE IF FURTHER ACTIONS ARE NECESSARY.
